I don't really think I can add much to Dr. Goldstein.  He's one of the
experts I would turn to when I needed help.  I can suggest that when you
sterilize all your equipment, that I would soak most of it overnight in a
bleach solution.  Also, throw out all gravel and other media and start with
fresh material.  Keep intermediate hosts in mind and keep some salt in each
tank.  Hope you get up and running again soon.
