What procedures are the experienced people out there using to quarantine fish?

I will collect all information received and submit a summary. Perhaps it could
be used as a future FAQ file.  

Questions I can think of are:

How much room do the quarantined fish need?  Typical 1 gallon per inch?

How long to quarantine before introducing fish to permanent tank?

What do you specifically look for in the quarantined fish?  What symptoms?

How do you set up a quarantine tank?  Bare tank?  Temperature control?  What
type of filtration?  How do you keep water cycled?

Any preventative treatments?
