Silly peat moss question

A silly question. When you use peat moss in a filter, does it get "used up" 
or does it simply disappear?

I have a filter bag full of peat granules (Fluval) in my filter and it 
seems to be getting harder. Is it picking up calcium from the water? Am I 
supposed to replace it every month or so, or just when it dissolves entirely?
