Two more possibilities: Perhaps it really is a clam. The first stage in 
life of a freshwater clam is parasitic. In our area you often see 
minnows in the spring covered with what looks like black "ick". The fish 
would have to have shared water with spawning clams since it's not 
contageous or that harmful. Or, it could be Cypris. Descriptions often 
refer to Cypris as clamlike in appearance, though not at all related to 
a clam since it's a crustacean.
