What color were the gills?

In nitrite poisoning (aka brown blood disease) one or a few weaker fish 
will often go down first. Salt is a very effective antidote.

The gills may look darker or brownish, instead of bright pink.

[That all others are healthy makes my guess pretty uncertain, tho.]
