A few useful tricks:
Slow the drill down. Larger diameter holes need lower RPM as it is the
surface speed that counts. Too fast just skids or hydroplanes without
grinding much.
Check for stress areas by looking at the region to be drilled between
two crossed polarizers. Stress areas show up as rainbow patterns instead
of solid black. Tempered glass (do not drill) has a rectangular pattern
of colors, usually.
I like a thick piece of glass as backing, and even wax the two together
if I want a really clean exit hole. [That's hard to do to a tank without
damaging the plastic rim with the heat used to melt harder waxes.]
As others have advised, keep pressure low but steady. Don't try to rush it.
