I think I am right in saying that the reaction between Fe and oxygen is direct. Where would the CO2 and H2O come from? The carbon in the packs does not ignite.

There should be no CO2 produced and I think any H2 production at the temperatures involved would be negligible. To try to settle all this, I emailed the distributor of the heat packs I use, but haven't heard back yet.

I wonder what the result is with an iron, oxygen and water reaction which
gives off hydrogen with breathable bags. Doesn't seem like you want hydrogen
going in the bags instead of oxygen.
