Re: 100% death.

I would suspect either the garden hose and/or runoff.  Most garden hoses 
have plasticizers and other toxins that can leach out and kill fish, 
which is why only hoses rated for drinking water should be used for fish 
projects.  Cost a bit more, but a lot safer. Many of the pesticides we 
use around our homes are also toxic, which is why one should not allow 
runoff from yards, etc. to drain to a pond.
