I have had similar experiences when I first started using yeast about
six months ago.  I did just as you have to remedy the situation - about
25% water change and sfc. agitation.  I never lost a fish, but I caught
it the same day I set up the DIY CO2.  I don't think you have to worry
too much. If the fish look good, then they probably are.
  Now to keep the yeast from overflowing into the tank, I leave about
two inches of air in the yeast bottle.  That way there is room for the
yeast to foam before it gets to the air line.
