I just saw a URI VHO fluorescent lamp which had several holes, some
about 1/8" dia., in its phosphor coating.  I'm not sure if this is
anything to worry about, but I thought I'd mention it because of the
possible UV hazard.  It was installed by a professional aquarium set up
and maintenance person and was only a couple weeks old, if that matters.

I guess it's something that should be checked with all fluorescent
lamps, regardless of brand.
