Reconstituting Water

Starting with RO water, I would like to end up with water with a GH 5 and KH 3.

I intend using Calcium Carbonate, Sodium Bicarbonate & a good quality sea
salt for the trace elements. Could someone give me an idea of what ratio to
use the above ingredients to obtain GH 5 & KH 3 ??

I have used Calcium Carbonate before to increase the GH but it does not
readinly disolve in water - what would be a better option and how much
should I use.
