Could it be that the water you use contains high levels of heavy metals like
copper of lead?
I was thinking about that because your water is very soft and has a neutral
pH. This can lead to increased solubility of metals. Some years ago, I had
massive fish kills when mixing 50 %rain water with tap water. It turned out
that the rain water dissolved some zinc from the rain collectors... the fish
did not appreciate the zinc.
