Your description doesn't include anything that would normally kill fish.  That
makes it hard to even speculate on what the problem was.

When you say that pH was off the chart high; what is normal? how high does the
chart go?

I'm confused by the idea that you added calcium carbonate and it brought the
pH down to 6.5.  Adding calcium carbonate should *increase* pH, not decrease it.

Did you notice any cloudiness in the water?  Were the surviving fish showing
any symptoms?  Was there anything common in the appearance of the dead fish?
