fin rot

what's the best way to treat finrot?  will melafix work?  and will this kill 
the filter bacteria?  what's the best way to prevent finrot? i vacuum the 
gravel of the tanks but i don't disturb the caves where the catfish live and 
i'm afraid the bacteria concentrate in those areas and are causing this 
problem.
