Your story reminds me of a Betta I lost in that exact same way when I first 
set up my paludarium. I would broaden your advice to include 'anything that 
has the slightest opening', not just an overflow box. In my case it was a
pile of lava rock in the sump. The fish got stuck in there and got so badly 
hurt that I couldn't save him from infection.
